Access Microsoft Forms by going to forms.office.com, and if prompted sign in with your Solihull credentials.
1. Click ‘New Form’ to launch the Form editor.
What’s the difference between ‘Form’ and ‘Quiz’?
A quiz requires correct answers to be established and are self-scoring.
A Form does not, and usually collects information that is not right or wrong.
2. Title your Form, and then press ‘+ Add new’ to create your first question.
Questions can be set as required – meaning that they must be filled out.
Images and videos can be inserted to be part of a question.
There are several types of questions that you can set a few examples and their descriptions are listed below.
Choice Respondent chooses one or multiple options from a list. Can be set up as a drop-down list also.
Text Respondent writes either a short or long response in a text box. Both long and short answers share the same character limit (4,000), however a long answer box will auto resize to show all the text as it is typed.
Rating Respondent votes on a rating that pertains to a statement.
Date Respondent chooses a date.
Linkert Respondent chooses one of five (or more) options that pertain to a statement.
Upload File Respondent is asked to upload a file from their own PC.
Ranking Respondent rearranges a selection of statements into a custom order.
Net Promoter Score Respondent sets a score in relation to how likely something is.
3. Continue adding questions until the Form is complete.
4. Use the style tab to change the Form’s aesthetics. (Optional)
5. Configure the Form’s Settings accordingly by clicking ‘…’ then ‘Settings’.

6. Your Form is now ready to be shared.
